Change of Circumstance Form for Post-Secondary Studies
Complete the Change of Circumstance form: 
- to provide additional information
- to change or clarify information that you, your parent(s) or spouse/partner have already provided on your original Application for Financial Assistance.
For example, use this form to advise Students Finance about the high cost of rent and utilities or unexpected medical expenses.
NOTE: Change of Circumstance forms are processed in the order they are received. You will receive a letter by mail after your form is processed advising you of the results. If you require further information or assistance after you have received your letter, contact your school, the nearest Alberta Service Centre or Canada-Alberta Service Centre or the Student Funding Contact Centre.
Millennium Alberta Rural Incentive Bursary - Schedule 3
Complete Schedule 3 if you:
- attended a rural high school in Alberta, or
- lived in rural Alberta for a minimum of 12 months immediately prior to starting post-secondary studies, and
- are eligible for a minimum of $1 000 in student loan funding.
Important: The Schedule 3 is included in the application package and must be attached to your original Application for Financial Assistance if you are applying by paper. The Schedule 3 will not be processed if it is sent to Students Finance without an application. However, if you have already submitted an Application for Financial Assistance and forgot to complete or attach a Schedule 3, then you may either mail your completed Schedule 3 (see address below) or fax to (780) 422 4516.
NOTE: If you are applying for funding online, you do not have to send in a paper Schedule 3.
Federal/Provincial Grant for Post-secondary Students with Permanent Disabilities - Schedule 4
If you are a student with a documented permanent disability and you are attending a full-time post-secondary program, you may be eligible to receive additional funding.
Consent to Disclose Personal Information to a Third Party
To grant another person access to your student financial information, complete the Consent to Disclose Personal Information to a Third Party form
For example, use this form if you are studying outside of Canada and you want Students Finance to disclose your student financial information to a parent or another person.
Should you require a third party to request action on your behalf, then you need to complete a Power of Attorney form.
Application for Interest-free Status for a Person on Parental Leave
If you take a break from full-time studies for parental leave, you may apply for interest-free status on your Alberta Student Loans.
Statutory Declaration for Interest-free Status for a Stepparent on Parental Leave
If you are a student who takes a break from full-time studies for parental leave as a stepparent, and you apply for interest-free status on your Alberta Student Loans, you may have to complete a Statutory Declaration form and attach to your application.
